A Request for More Information might seem like a minor setback. You get 30 days to fix it, right? The problem is that these 30 days can cascade:

  • You discover the fix takes longer than expected
  • You miss the Request for More Information deadline
  • DHS issues a 60-day termination notice
  • Your enrollment is terminated
  • You face a 6-month freeze before you can re-apply

Avoiding these mistakes in the first place is critical. Here's what to watch for:

Mistake #1: COI Date Mismatch

Most Common

The Problem

Your Certificate of Liability Insurance (COI) start date is AFTER your service start date. This seems like a minor administrative issue, but DHS treats it as a compliance failure.

Why It Happens

Insurance policies renew on their own schedule, not aligned with your MHCP enrollment. When you upload your current COI, the effective date might be recent—but your services started years ago.

The Fix

Contact your insurance provider and request documentation showing continuous coverage from your original service start date. Many providers can issue a certificate showing "Continuous coverage since [original date]" or provide historical COIs.

Mistake #2: Missing DHS-5550 for Ownership Changes

Causes Automatic Reset

The Problem

Any ownership or management change since your last revalidation requires a DHS-5550 Sale or Transfer Addendum. Many providers don't realize this applies to them.

Why It Happens

"Ownership change" doesn't just mean selling your business. It includes:

  • Adding a new owner (even a small percentage)
  • An existing owner's percentage changing
  • An owner leaving or being bought out
  • Changes to management company arrangements
  • Corporate restructuring

The Fix

Review your ownership structure from your last revalidation date to now. If ANYTHING changed—even if you think it's minor—include the DHS-5550 with your submission. It's better to include it unnecessarily than to miss it.

Mistake #3: Incomplete Ownership Disclosure

Frequent Cause of Request for More Information

The Problem

DHS-3891 requires disclosure of ALL owners with 5% or more direct OR indirect ownership. Many providers miss the "indirect" part or don't fully understand complex ownership structures.

Why It Happens

Indirect ownership can be confusing. If your company is owned by another company, the individuals who own that parent company may need to be disclosed. This applies even if they're not directly involved in operations.

The Fix

Create an organizational chart showing all ownership relationships. Ask yourself: "If we trace ownership up the chain, who ultimately owns 5% or more of this provider?" All those individuals need disclosure and background studies.

Mistake #4: Background Study Issues

Cannot Be Rushed

The Problem

One or more 5%+ owners don't have an eligible fingerprint-based background study on file. You cannot submit revalidation without this—and background studies take 2-3 weeks to process.

Why It Happens

  • New owners were added but background studies weren't completed
  • Background studies expired
  • Studies were done through wrong system (not NETStudy 2.0)
  • Status shows "Pending" instead of "Eligible"

The Fix

Check background study status for EVERY 5%+ owner TODAY. If anyone shows anything other than "Eligible," start the background study process immediately. This is the one issue you absolutely cannot rush if you wait too long.

Mistake #5: Expired Training Certificates

Easily Preventable

The Problem

Required training like Waiver Provider 101 has expired, or TrainLink transcripts are out of date.

Why It Happens

Training requirements have ongoing renewal periods that don't align with revalidation schedules. It's easy to lose track.

The Fix

Pull your TrainLink transcripts and check expiration dates for all required training. If anything expires within 60 days, complete the renewal now before you submit. Some training can be completed online in a few hours.
