Understanding the Revalidation Process
Minnesota MHCP high-risk provider revalidation isn't a single form you fill out. It's a multi-step process involving document gathering, portal submission, state review, and a site visit. Understanding each step helps you avoid costly delays.

Step 1: Check Your MN-ITS Mailbox
Before you can revalidate, you need your official notice from DHS. Here's how to find it:
- Log into your MN-ITS account
- Navigate to your mailbox
- Look in the "PRVLTR" folder
- Find the letter from DHS about high-risk revalidation
The notice window is January 23-28, 2026. If you don't see your notice by January 29, contact the MHCP Provider Resource Center at 651-431-2700.

Step 2: Gather Required Documents
This is where most providers run into trouble. Missing or outdated documents cause immediate Requests for More Information that delay your approval.
Document Checklist
List all owners with 5%+ direct or indirect ownership interest. Include full legal names, SSNs, DOBs, and ownership percentages.
Must show coverage effective dates. Critical: The start date must match or precede your service start date.
Screenshot or printout showing "Eligible" status for all 5%+ owners. Studies must be fingerprint-based.
Waiver Provider 101, TrainLink transcripts, and any service-specific required training.
Federal Employer Identification Number verification.
Supplier ID and Location Code for electronic payment.
Required if ANY ownership or management changes occurred since last revalidation.
Step 3: Complete Background Studies
Background study issues are the #1 cause of revalidation delays. Here's what you need to know:
Who Needs a Background Study?
All direct and indirect owners with 5% or more ownership interestmust have a current, eligible fingerprint-based criminal background study.
Where to Submit Background Studies
Most high-risk provider types use NETStudy 2.0:
- Adult Day Care
- CFSS & PCA Provider Organizations
- Companion Care
- Early Intensive Developmental and Behavioral Intervention
- Individualized Home Supports (IHS)
- Integrated Community Supports
- Intensive Residential Treatment (IRTS)
- Night Supervision
- Non-Emergency Medical Transportation (NEMT)
- Substance Use Disorder (Peer Recovery)
Some provider types don't have NETStudy 2.0 access yet:
- Adult Rehabilitative Mental Health Services (ARMHS)
- Assertive Community Treatment
- Peer Recovery – Recovery Community Organizations
- Recuperative Care
For these types, contact DHS directly for background study instructions.
Critical Timing Issue
Background studies can take 2-3 weeks to process. If your owners don't have eligible studies on file, you need to start this immediately. You cannot submit revalidation without eligible background studies.
Step 4: Access the MPSE Portal
All revalidation data entry and document uploads happen through the MPSE (Minnesota Provider Screening and Enrollment) portal.
Accessing Your Enrollment Record
- Log into MN-ITS with your organization credentials
- Navigate to the MPSE section
- Find your enrollment record for the location being revalidated
- Look for the revalidation action or "Update" option
What Data You'll Enter
- Owner information (all 5%+ owners)
- License and credential details
- Insurance information
- EFT/payment details
- Service location information
Document Uploads
You'll upload PDF copies of all required documents. Make sure files are:
- Clear and legible
- Complete (all pages)
- Current (not expired)
- Properly named for easy reference
Step 5: Pre-Submission Audit (Critical)
Before you click submit, perform this pre-flight check. These are the exact issues that cause Requests for More Information and denials:

Step 6: Pay the State Application Fee
You cannot submit revalidation without paying the state fee. Here's how it works:
- Amount: $750 per location (effective January 1, 2026)
- Paid to: DHS (not to any third party)
- How: Through the DHS Payment System
- Confirmation: You'll receive a confirmation number (format: MN2DHS...)
After payment, you'll enter the confirmation number into the MPSE portal before final submission.
Step 7: Submit and Track
Once you've completed all steps and paid the fee, click "Final Submit" in the MPSE portal. You'll receive a submission receipt.
What Happens Next
- DHS has 30 days to review your submission
- They may send a Request for More Information if anything is missing
- You have 30 days to respond to any Request for More Information
- After document review, an unannounced site visit will occur
- Revalidation is complete when you receive the "Revalidation Complete" letter
Tracking Tip
Check your MN-ITS mailbox "PRVLTR" folder regularly. Set a calendar reminder for Day 15 and Day 25 after submission to check for updates.
Step 8: Prepare for Site Visit
After your documents are approved, DHS will conduct an unannounced site visit. "Unannounced" means you won't know exactly when—so be ready at all times.
Physical Environment Checklist
- 245D license posted in visible area (if applicable)
- Records stored behind two locks (physical security)
- Emergency contact information posted
- Required signage displayed
Documentation Ready
- Current organizational chart printed and accessible
- Employee background study files accessible within 5 minutes
- MHCP Provider Agreement printed and signed on-site
- Training records (TrainLink transcripts) organized
Staff Preparation
- Designated person identified to greet inspector
- Manager knows where ALL required documents are located
- Staff trained on appropriate responses
If DHS Doesn't Respond in 30 Days
If you haven't heard anything 30 days after submission, you need to proactively follow up:
- Call the MHCP Provider Resource Center: 651-431-2700 or 800-366-5411
- Request to open a case for your revalidation
- DHS then has 5 business days to respond
Don't assume no news is good news. Follow up if the timeline passes.
