  • File sampling is used to identify patterns and system gaps that drive repeat findings. You can choose a provider-selected sample, or request random selection. Expanded scope is available if you want additional files reviewed.

  • Often, yes - repeat gaps typically show up as patterns across systems and staff habits.

    If you want greater certainty or you have complex operations, you can add more files per the add on rates.

  • Yes. If there are “problem files,” request a provider-selected sample and flag them during intake.

  • Not by default. Packages include a defined file sample size. If you want a larger scope, additional files can be added.

  • Note it in the intake form. Multi-site or higher-volume operations may require a custom scope and quote.

  • If something isn’t maintained, label it “Not maintained.” when you upload your documents. We’ll document the gap.

  • Package prices are fixed for the included scope. Final cost can change only if scope expands (additional files, Intensive add-on, rush timeline, multi-site complexity).

  • We provide findings and a corrective action plan when included. Your agency implements the corrections. Comprehensive includes a debrief to align leadership on priorities and sequencing.
